Consent Forms

Question from a Leader: "If parents share their profiles with the Club using Caremonkey, do we still need to use the paper Consent Forms?"

The short answer is: no. Using Caremonkey, this would be your process: 1. Application & Parent Agreement 2. Upload profile requests to Caremonkey 3. Parents share the requested profiles 4. Create eForm for your Club from template: 2017 Club General Consent 5. Send out to your members in Caremonkey for them to sign Repeat steps 4 & 5 using the event template for any offsite or overnight events during the year. Using this process you will not require the Consent Form or Additional Medical Info form.

    Screen shots are included in the comments herewith to step you through the process of adding Profile Requests. Note: As a Club Administrator you are not adding Care Profiles - profiles are owned and managed by the parent/guardian. You are just sending a request to the parent/guardian to view the child's profile (or to the volunteer to view their profile).
